It is years of incompetence and negligence snowballing into this tragedy. Yup. You hear horror stories of people in the industry that somehow keep getting work despite that incompetence and negligence because they've been in the game for a period of time. You would think after multiple incidents these people wouldn't get rehired, but surprisingly they always seem to find their way back onto a set. This needs to be a wakeup call to the industry on vetting crew members a lot more seriously than they have been. From what I'm hearing the era of streaming and "peak tv" means that significantly more (deep sigh) content is being made than ever before and that is making it so the talent pool of competent and experienced crew members is increasingly stretched thin, meaning people like this get promoted faster and get more work.

That makes a lot of sense. This is just an anecdote, but a roommate of one of my friends works in the newly formed department of COVID compliance on sets. They basically test everyone to make sure they're not sick and also serve as sort of ADs/PAs whose sole function on set is making sure people are being safe and adhering to COVID guidelines. Well, that roommate is about the absolute last person you want on set making sure people are safe and healthy. He's told me on numerous occasions that he reports to work stoned, smokes weed on set, and mostly hangs around craft services until it's time to go home. Not only not doing his job, but serving in a position of monitoring safety on set while being a complete hazard to everyone around him. He's been fired from reality shows on multiple occasions, yet gets hired onto something new the very next week. Not to mention that his position pays good money. It's made me on several occasions wonder aloud if they're just hiring anyone for some of these positions, and if he's any indication the answer is yes.
