That was the most fucked up scene in the movie, not just because he put his thumb in her mouth, but because of the way he manipulated her and played with her emotions. It's really the most important scene in the movie. The whole premise is that an ex-convict taunts a lawyer through legal loopholes. It's easy for the audience to side with De Niro because the concept and the character are so hilarious, and the Nick Nolte character ain't no saint either, so there really needed to be a scene where the audience is reminded that he's a monster.

It's easy to forget what a major departure this was for Martin Scorsese. Not only because he's dabbled in action and special effects in the years following Cape Fear, but also because the commercial film landscape has changed. Movies like this don't make money anymore unless the horror aspect is played up and the cast were considerably younger.
Cape Fear is a solid thriller and proves that Scorsese could have been a major "Hollywood" filmmaker if he had wanted to. He knows what audiences want and has enough style and personality to distinguish his work from the rest of the pack.
I wouldn't trade quintessential Scorsese for this, but I'd rather have this Scorsese than the one who made nonsense like Age of Innocence, Kundun and some of the 21st century ones. Keep it dark, modern and real. That's when you're at your best, Marty.

I rewatched Cape Fear recently too. Didn't write much, but I'll share what I wrote.
Still love it. Martin Scorsese's Cape Fear remake is style over substance and not one of the director's must-sees, but man is that style engrossing and what really elevates what is already a rock-solid thriller. Robert De Niro's batshit performance is also really effective and I really like the way Max Cady unnerves issues that already existed within the family. It also should be noted that while this is style over substance, Cape Fear is not all flash. There are some challenging dynamics to the conflict at hand which add some depth to Cape Fear. Not a lot, and depth which is overshadowed by the excess and filmmaking, but still.
